UPDATE: Seems the first aid kit isn't available to everyone. Here is the e-mail I received:
"While the online quiz is open to the public, first aid kits will only be mailed to addresses within the Avera service area (South Dakota, Iowa, Minnesota and Nebraska.)"
They should really have this entered on their website for all to see.
