<--Just a couple of things with this one. As you can see in the FYI, this is good for the first 4,000 unique entries only. I hate it when they don't tell you how many are left before the samples are gone. They leave the offer up to get further e-mail addresses They should just remove the offer. Hopefully all of you aren't using your main e-mail address. ALSO, just above the entry form you will see:

"No purchase is necessary to enter. If you'd prefer to receive your free sample without starting a trial subscription to Harper's BAZAAR or if you are already a subscriber, click here."

Make sure you click or you will find yourself paying for a subscription to Harper's BAZAAR.

Name/Address
E-mail
Phone
Three opt in boxes
